I have to say that scruffy shoes is my biggest pet peeve, and you could include all the square toe monstrosities along with that. Along with the phone/pda belt clip thing.
I would be interested to hear opinions on this article from SF'ers that work in more of a business/financial/etc environment...

Yeah....too bad they didn't talk to somebody in charge of marketing for the Tide pen. He might have a different opinion. However, I do like the idea of keeping a spare white or light blue shirt in the office for emergencies. If I got a stain on the way to a high-level client meeting or interview, I would replace if possible. I wouldn't throw out the original, just bring to a dry-cleaner.
