Answer:
I would try this hard to find area called "The Fashion District"
Answer:
I have always here "Tenderbuttons".
As an aside, it invariably cracks me up when poster ask for recommendations for this or that, in NYC. I mean, come on, if you can't find it in NYC, it probably does not exist.
Answer:
Yes, Tender Buttons (which, btw, means nipples) is a good source. A store devoted entirely to buttons. I buy horn buttons there for suits and sportcoats when they are missing, or suck.
Answer:
According to Tender Buttons, the thickest shirt buttons they sell are 1/16". I asked for the conversion to millimeters, and they couldn't tell me. According to my math, 1/16" is less than half the thickness of a 3.5mm button.
Answer:
I've seen them on eBay.
Answer:
AK sells nice ones: http://www.customshirt1.com/PearlButtons.htm .

Answer:
Seriously, pretty much any store in the west 40's that sells fabrics, buttons etc will have what you are looking for.
Answer:
you will overpay in the garment district for MOP buttons. You can get great odd buttons at many stores. In fact I just picked up a set of buttons for a vintage overcoat. I think I paid $1.00 for 12.
